There are 3 methids.
1. Percussion of traube.s space or beneath's space.
Stand on the right side of the patient, make the patient in supine position, find the traube's space as shown in video. Percuss over the area using your fingers. If dull note heard then possible splenomegaly is suspected. However a left sided or bilateral pleural effusion might give a false positive result. This method is preferable then palpation as for a clinically detectable splenomegaly by palpation it should enlarge 2-3 times.
2. Percussion by castell's method.
Simply purcuss over 8th & 9th intercostal space along the left anterior axillary line while during inspiration.
3. Percussion by nixon's method.
Ask your patient to lie in right lateral position. Percuss from below upwards along an imaginery line perpendicular to middle of left costal margin heading towards left axilla, stop till you get a dull note and mark it. Measure the distance from this dull point to the midpoint of left costal margin.
If the distance is more than 8 cm then possible spleen is normal. Otherwise normal.
